Formalized mechanisms
for error management are often not used. This means that incidents
(events which are assumed to be errors) are reported in an ad hoc
and often prosaic manner. The link from requirement to code to test
is not well maintained and it is often difficult to know if a real
error had been reported and subsequently fixed.
The inappropriate
control of error reporting results in an inability to plan and resource
an appropriate level of support, and leads to errors sliding through
the Test net and into the client site.
